29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. a) Worksite, 105 Emjay Blvd, Brentwood, NY - An employee operating forklifts including but not limited to a TOYOTA Electric Forklift Truck and YALE Lift Truck, was not evaluated in the last three years, on or about 02/27/2019. b) Workplace, 85 Nicon Ct., Hauppauge, NY - An employee operating forklifts including but not limited to YALE Lift Truck, YALE Lift Truck, Mitsubishi Lift Truck and a YALE Lift Truck, was not evaluated in the last three years, on or about 04/25/2019.
