This was most recently documented on or about 01/03/19 at the establishment located at 3608 W. 30th St. South, Wichita, KS. The operator and other employees working/walking in the area were exposed in that an evaluation of the operator's performance was not conducted at least once every 3 years to ensure the operator was able to operate the Nissan Forklift (KPH01A15V) safely.
