29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: The employer is failing to protect employees from struck-by and fall hazards. This was most recently documented on or about October 17, 2018 at the residential construction jobsite located at 522 Iowa Ave., in Holton, Kansas. An employee was fatally injured when the all-terrain fork truck overturned. Employees engaged in re-roofing activities were lifted in/working from a work platform attachment on the fork truck. The work platform attachment was not approved for use on the TEREX SS-842C all-terrain fork lift by the manufacturer.
