Text For Citation: 01 Item/Group: 006 Hazard:
29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Army Radford Ammunition Plant Building 1626: On or about 06/11/2018, an employee was not provided with an aluminized suit when disturbing propellant that reacts violently to friction, shock, impact and electrostatic discharge. An employee was required to push propellant off a discharge cone, through a discharge chute and into a drop plug buggy using a wooden paddle. b) Army Radford Ammunition Plant Building 1626: On or about 06/11/2018, employees were not provided with the appropriate FR coveralls when working with propellant that reacts violently to friction, shock, impact and electrostatic discharge. An employee required to wear coveralls stood on the breezeway located outside of Bay #4 received second and third degree burns to the body. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
