29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed: a.) On or about May 31, 2018, the employer did not ensure a lock was placed on the breaker panel while replacing a 15amp fuse on the P3 paint line conveyor exposing them to electrical hazards from unexpected start up of the equipment.
