29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) The employer did not ensure that each powered industrial truck operator has successfully completed the training program consisting of a combination of formal instruction, practical training, and evaluation. This violation occurred on or about May 9, 2018 and at times prior thereto; when employees were operating the forklifts without having formal instruction, practical training and evaluation.
