29 CFR 1910.134(e)(6)(i): The employer did not obtain from the physician or other licensed health care professional (PLHCP) a written recommendation regarding the employee's ability to use the respirator: a) Atlantic Precast Concrete - On or about March 20, 2018, an employee working in the Yard was provided a half face elastomeric cartridge respirator for use while drilling holes into concrete, and the employer did not obtain a written recommendation from the PLHCP regarding the employee's ability to do so. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
