29 CFR 1910.140(d)(2)(i): The employer must ensure on any horizontal lifeline that may become a vertical lifeline, the device used to connect to the horizontal lifeline is capable of locking in both directions on the lifeline. a)On January 9, 2018 located at 3745 Beechmont Ct. Cincinnati, OH 45226 (Jacob Brothers Heating & Air Conditioning) employees performed sign installation associated operations on upper and lower back unprotected side or edge of Bulletin sign #5502 stage/platform utilizing a personal protective fall arrest lanyard connected to a horizontal lifeline which may become a vertical lifeline on which the employer did not ensure the lanyard used to connect to the horizontal lifeline was capable of locking in both directions on the lifeline exposing employees to fall potentials in excess of 25 feet. b) On January 9, 2016 located at 3745 Beechmont Ct. Cincinnati, OH 45226 (Jacob Brothers Heating & Air Conditioning) employees performed sign installation associated operations on lower front unprotected side or edge of Bulletin sign #5502 stage/platform utilizing a personal protective fall arrest lanyard connected to a horizontal lifeline which may become a vertical lifeline on which the employer did not ensure the lanyard used to connect to the horizontal lifeline was capable of locking in both directions on the lifeline exposing employees to fall potentials in excess of 25 feet. c) On January 22, 2018 located at 3745 Beechmont Ct. Cincinnati, Oh 45223 (WAE Church) employees performed sign installation associated operations on upper back unprotected side or edge of Bulletin sign #1192 stage/platform utilizing a personal protective fall arrest lanyard connected to a horizontal lifeline which may become a vertical lifeline on which the employer did not ensure the lanyard used to connect to the horizontal lifeline was capable of locking in both directions on the lifeline exposing employees to fall potentials in excess of 13 feet. e) On March 6, 2018 located at 1814 Powers Ct. Cincinnati, Oh 45223 (WAE Church) employees performed sign installation associated operations on lower front unprotected side or edge of Bulletin sign #1192 stage/platform utilizing a personal protective fall arrest lanyard connected to a horizontal lifeline which may become a vertical lifeline on which the employer did not ensure the lanyard used to connect to the horizontal lifeline was capable of locking in both directions on the lifeline exposing employees to fall potentials in excess of 13 feet. f) On January 30, 2018 located in the back of a parking lot between 2325 & 2335 W. Mc Micken, Cincinnati OH 45214 employees performed upper back sign installation associated operations on upper back unprotected side or edge of Bulletin sign #1312 stage/platform utilizing personal protective fall arrest lanyard connected to a wire rope mid rail / horizontal lifeline which may become a vertical lifeline on which the employer did not ensure the lanyard used to connect to the horizontal lifeline was capable of locking in both directions on the lifeline exposing employees to fall potentials in excess of 15 feet. g) On January 12, 2018 located adjacent to old Norwood incinerator at 3001 Duck Creek Road Norwood Oh 45212 employees performed upper back sign installation associated operations on upper back unprotected side or edge of Bulletin sign #5028 stage/platform utilizing personal protective fall arrest lanyard connected to a horizontal lifeline which may become a vertical lifeline on which the employer did not ensure the lanyard used to connect to the horizontal lifeline was capable of locking in both directions on the lifeline exposing employees to fall potentials in excess of 20 feet. h) On January 12, 2018 located adjacent to old Norwood incinerator at 3001 Duck Creek Road Norwood Oh 45212 employees performed sign installation associated operations on lower front unprotected side or edge of Bulletin sign #5028 stage/platform utilizing personal protective fall arrest lanyard connected to a horizontal lifeline which may become a vertical lifeline on which the employer did not ensure the lanyard used to connect to the horizontal lifeline was capable of locking in both directions on the lifeline exposing employees to fall potentials in excess of 9 feet. i) On January 5, 2018 located beside 1400 Reading Rd. Cincinnati, Oh 45202 (Staples) employees performed sign installation associated operations on lower front unprotected side or edge of Bulletin sign #1407 stage/platform utilizing platform utilizing personal protective fall arrest lanyard connected to a horizontal lifeline which may become a vertical lifeline on which the employer did not ensure the lanyard used to connect to the horizontal lifeline was capable of locking in both directions on the lifeline exposing employees to fall potentials in excess of 12 feet. j) On January 5, 2018 located beside 1400 Reading Rd.
