29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards while cleaning as floor hole covers on the third floor to the north and east sides of the Townhouse, were not properly secured or covered to prevent employees from trip / fall hazards. Violation was observed on or about December 11th, 2017. b) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards while cleaning and working on the northeast and northwest stairs as floor hole covers on the fourth floor to the north and east sides of the Townhouse, , were not properly secured or covered to prevent employees from trip / fall hazards.
