29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed: Workplace, 55 Hartz Way, Secaucus, NJ a) The employer did not ensure that employees changing out the control switch for the Inductor Melter and Furnace VIP 50 Panel locked and tagged out 440 Volt circuits prior to opening the panel and replacing the switch. Employees performing the work only turned the control circuit breaker off and did not apply a lock and tag indicating it should not be energized. Violation occurred on or about 12-5-17
