29 CFR 1926.1060(a)(1)(v): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the standards contained in Subpart X: On or about September 25, 2017 located at 11332 Tamarco Dr. Blue Ash OH along a front overhead doorway entrance employees were gaining access to the metal decked roof area of the structure by utilizing a fiberglass extension ladder which was not secured from displacement or protected by barricade exposing employees to fall potentials in excess of 21 feet who were not trained on the applicable standards.
