29 CFR 1926.1204(d)(7): The employer did not provide equipment such as ladders, needed for safe ingress and egress by authorized entrants: (a) Corner of 7th and South Street located in Petal, MS - On or about June 13, 2017 the employees used the Maintainer work truck crane and a job-made swing to enter and exit the sewer waste water lift station. The job-made hoisting system was not approved for personnel hoisting by a registered professional engineer, prior to use.
