29 CFR 1926.800(f)(1): When natural unassisted voice communication was ineffective, a power assisted means of voice communication was not used to provide communication between the work face, the end of the shaft, and the surface: a) Culvert Install Site: On or about May 05, 2017 and at times prior, employees were exposed to acute illness, burns, impairment of ability to self-rescue, and incapacitation while removing soil, inspecting, flame cutting, tacking and welding inside of a 36 inch culvert without adequate communication.
