29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c). On or about 15 April 2017, and at times prior thereto, two workers were performing work within a trench that was over 7 feet in depth without a protective system in place, exposing them to the hazard of being caught in a cave-in or engulfment.
