29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about February 21, 2017, in buildings 1 and 2, employees were required to operate powered industrial trucks to move material and products such as hunting blinds and feeders without first receiving training from the employer. Employees are also being lifted by the PIT's to work from an elevated height without fall protection. Employees were exposed to the hazards of being-crushed between the forklift and ground in the event of a tip over and falls.
