29 CFR 1926.451(e)(1): Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platforms more than 2 feet above or below a point of access: a) On or about February 4, 2017, employees were exposed to a fall hazard of approximately 6 feet while accessing the work platform of a tubular welded frame scaffold.
