29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On or about February 1, 2017, and time prior to, in the Waste Water Treatment area, the exposed conductors in the electrical outlet suspended above the Batch Table were not covered. This condition exposed employees to the hazard of contact with live parts and electrical shock.
