29 CFR 1926.1428(a): The employer of the signal person did not ensure that each signal person met the Qualifications Requirements (paragraph (c) of this section) prior to giving any signals: a) Construction site, Building 100 - On or about January 6, 2017, The employer did not ensure and certify that each signal person met the requirements of paragraph C of this section. Employees gave hand signals to a crane hoisting trusses and landing them in an area the operator could not see.
