29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) Newman Box - On or about November 30, 2016, each authorized employee performing clearing a jam in the Newman Shaver 1 & 2 did not affix a personal lockout or tagout device to the group lockout device before working on the machine. The procedure did not include controlling the hazardous energy of the Newman Log Feed Conveyor.
