29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) At the facility, on or about and at times prior to November 29, 2016, the employer allowed employees to operate a Toyota propane forklift (Model: 8FG1318 S/N: 10595) with missing rubber and severe cracks on the edge of the tires, exposing employees to crush-by and struck-by hazards.
