29 CFR 1926.451(g)(1)(i): Each employee on a ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 09/19/16 at 375 Melville Street, Rochester, NY; the employer did not ensure that each was protected by a personal fall arrest system when employees removing roofing materials from the dormer on the west side of the house were on a ladder jack scaffold at a height of approximately 15 feet when one of the ladders collapsed and three employees fell to the ground without the use of a personal fall arrest system. ABATEMENT DOCUMENTATION REQUIRED
