29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors are not replaced: Employees performing welding activities are exposed to electrical shock and burns and the employer failed to replace cables that were damaged. This violation was last observed on September 6, 2016, on the MIllermatic 252 MIG welder, with the damaged grounding cable insulation, in the fabrication shop. To abate this violation, the employer must ensure that all damaged cables are replaced. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including a copy of site-specific program detailing the procedures used to ensure compliance and training of employees.
