Excavation Site: On or about July 20, 2016, the employer or his designee did not conduct soil classification tests in order to determine the maximum allowable slopes, and allowable configurations for sloping and benching systems in accordance Appendices A and B to this subpart. This condition exposed employees to cave-in hazards.
