29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: On or about 6/2/2016 in the equipment/chemical area of the facility located at 2434 West Broad Street in Athens, GA: The ventilation fan blades in the employee(s) work area were not guarded to protect the employee(s) being exposed to an amputation hazard. On 5/28/16 two of an employees fingers were amputated by the unguarded rotating ventilation fan blades.
