29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) On or about 05/23/2016 in the back bay area; where employees attach a parts basket to the forks of a powered industrial vehicle to access the fluid reservoirs on the hydraulic press approximately 12-15 feet above the concrete floor below. The parts basket is not an approved attachment. ABATEMENT CERTIFICATION REQUIRED
