29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: a. Stock Casting Area: On or about February 23, 2016, and at times prior employees were exposed to Methylene bisphenyl isocyanate (MDI) while performing stock casting operations utilizing Fibre Glast Part #625 Mix and Pour Foam (A Side). The employer did not ensure that adequate housekeeping practices were in place to remove surface accumulations of Methylene bisphenyl isocyanate.
