29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) 223 Grand Street, first floor, southeast corner, on or about 1-15-16: Employees were using a stairway that had a section removed from it, exposing the employees to a fall hazard of 17 feet 7 inches up to 20 feet 3 inches to the basement below.
