29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) On or about 01/06/16, in the Coal Handling Building; employees were exposed to fire, explosion and struck by hazards when the cover for the 1 Elevator and 2 Elevator disconnect, located on the Mix Table Floor, was missing 15 out of 20 bolts which secured the cover. ABATEMENT CERTIFICATION REQUIRED
