29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a.) Edge Plastics, Inc. located at 449 Newman St. Mansfield, OH 44902; On or about December 18, 2015, the employer did not ensure employees were protected while performing service and maintenance activities on Injection molding press #16. Employees were required to perform servicing activities inside the molds and did not place a lock on the disconnect switch, exposing them to crush hazards.
