29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Sawmill: On or about October 21, 2015, and times prior, an energy control program was not established for employees required to perform service and maintenance on equipment, such as, a 56 inch diameter head saw with hydraulic and 440V sources of energy, a Debarker with hydraulic and 440V sources of energy, a 36 inch Tower Edger with a 220V source of energy, a 32 inch Cutoff Saw with a 220V source of energy, a Chipper with a 220V source of energy, and a Vance Gang Saw with a 220V source of energy. (b) Planer: On or about October 21, 2015, and times prior, an energy control program was not established for employees required to perform service and maintenance on equipment, such as, a Vance Planer with a 220V source of energy.
