Text For Citation: 01 Item/Group: 006 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) At the time of the inspection, employees were using the Ersham elevator to access the upper levels of the west silos. The following deficiencies were found; 1. There was no rated capacity information available. 2. The suspension consisted of one wire rope. 3. There were no tests or inspections of lift. Failure of the elevator would create a fall from elevation hazard. Among other methods, one feasible and acceptable method to abate this hazard is to comply with ASME17.1-2004. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
