29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) The employer did not ensure that energy isolating devices that are needed for the control of hazardous energy were utilized on equipment such as, but not limited to, the Milnor dryer and Pellerin Milnor conveyor. On or about August 13, 2015, an employee was propelled into an industrial dryer when the conveyor system activated. The dryer subsequently activated with the employee trapped inside. (b) The employer did not ensure that energy isolating devices that are needed for the control of hazardous energy were utilized on equipment such as, but not limited to, the Milnor dryer and Pellerin Milnor conveyor. On or about August 11, 2015, an employee was conducting maintenance work and was not using the proper hazardous energy control devices.
