29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. Wilbert, Inc. - Bellevue, OH: On or about February 2, 2015, the employer did not develop energy control procedures to control hazardous energy when employees maintained or setup plastic molding machines. Employees that performed set-up, cleaning, or and/or troubleshooting of the A57 Haitian press were exposed to the unexpected startup of the machine and associated robot equipment. b. Wilbert, Inc. - Bellevue, OH: On or about May 14, 2015, the employer did not develop energy control procedures to control hazardous energy when employees setup plastic molding machines. Employees that performed set-up of the A56 Haitian press were exposed to the unexpected startup of the machine's associated robot equipment.
