29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): ESTABLISHMENT: The points of operation of the Machine N. 4, W&H Triumph 3G, and Machine No. 2, W&H Triumph 5A, were not guarded by appropriate guards and/or devices, exposing employees to rotating hole punchers and rotating rollers while re-feeding papers and paper handles. 1. On or about 04/30/2015, an operator's right hand was caught by the traction roller while the employee was trying to re-feed a paper handle piece through the paper handle assembly point of operation area of the Paper Converting No. 4 W&H 3G Machine, and the employee sustained the loss of the thumb from the incident. 2. On or about 08/15/2015, an operator's right hand was caught by the traction roller while the employee was trying to re-feed a paper handle piece through the paper handle assembly point of operation area of the Paper Converting No. 2 W&H Machine, and the employee sustained the loss of all of fingers of his right hand.
