29 CFR 1910.146(d)(8): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not designate the persons who were to have active roles in entry operations, identify the duties of each such employee, and did not: On or about April 3, 2015, VCNA Prairie, Inc. did not designate an entry supervisor to determine if entry conditions were acceptable prior to authorized employee entry into the batch plant mixer dump hopper and designate an entry attendant to monitor entry conditions during an authorized employee entry into the batch plant mixer dump hopper. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
