29 CFR 1910.146(d)(8): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not designate the persons who were to have active roles in entry operations, identify the duties of each such employee: 1.) On or about April 15, 2015, the employer did not designate who would fill required roles, such as but not limited to, entrant, attendant, supervisor or person to test the atmosphere during the confined space entry. Employees were entering the Trans Mat parts washer to perform work, such as but not limited, cleaning nozzles and performing maintenance without having jobs designated in the process. Employees are exposed to caught in hazards from the chain conveyor, chemical hazards from chemicals such as but not limited to; Potassium Hydroxide and Phosphoric acid and water engulfment.
