29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On and before March 6, 2015, located in the rear of the bowling alley for each of the forty (40) bowling lanes, employees walked and worked on the floor at the rear of the bowling lanes and were exposed to rotating parts and flying bowling pins from the unguarded entrance (elevator) of each machine.
