29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. Tiffin Metal Products Co. located in Tiffin, Ohio: On or about March 26, 2015, the employer did not ensure the Atlantic 17 foot press brake located in the Building #1 fabrication department was properly guarded, exposing employees fingers to pinch point hazards created between the upper die and the work piece. b. Tiffin Metal Products Co. located in Tiffin, Ohio: On or about March 1, 2015, the employer failed to ensure the Piranha ironworker located in the Building #1 "Pit" was properly guarded, exposing an employee's fingers to pinch point hazards created between the top die and the work piece. c. Tiffin Metal Products Co. located in Tiffin, Ohio: On or about February 25, 2015, the employer failed to ensure the RAS 74.40 panel folder was properly guarded, exposing employees fingers to pinch point hazards created between the top die and the work piece.
