29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment: a.) Shiloh Industries, Inc. dba Wellington Stamping located at 350 Maple St. Wellington, OH 44090; On or about February 5, 2015, the employer did not assure a program of periodic and regular inspections of the OBI Danly Mechanical Power Presses were established, completed, and certified. Employees were exposed to amputation hazards from bypassed safety features, top stop malfunctions, and no keys for the selector switches.
