29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) At Hawthorne Army Depot Lockout-Tag-out Procedures BOP.IOP.MUO.0208 was not followed, there were no evidence that periodic and/or inspections were conducted. b) In Building 63, the lockout/tag-out (LO/TO) station locks were color coded red, the lock used on the cargo elevator switch was a master brand padlock, the tag on the cargo elevator switch was dated 2-9-11.
