29 CFR 1926.1431(h)(1): A trial lift with the unoccupied personnel platform loaded at least to the anticipated lift weight was not performed from ground level, or any other location where employees will enter the platform, to each location at which the platform is to be hoisted and positioned. Where there is more than one location to be reached from a single set-up position, either individual trial lifts for each location, or a single trial lift, in which the platform is moved sequentially to each location, must be performed; the method selected must be the same as the method that will be used to hoist the personnel: (a)Northland Concrete and Masonry Company, LLC at 1508 West 22nd Street, Sioux Falls, SD 57105: On or about and at times prior to May 25, 2023, the exposing employer did not ensure a trial lift was performed involving the unoccupied Doka platform before personnel were hoisted. This condition exposed employees to fall, crushing, and struck-by hazards. (b)Northland Concrete and Masonry Company, LLC at 1508 West 22nd Street, Sioux Falls, SD 57105: On or about and at times prior to May 25, 2023, the exposing employer did not ensure a trial lift was performed involving the unoccupied man-basket before personnel were hoisted. This condition exposed employees to fall, crushing, and struck-by hazards.
