29 CFR 1910.261(j)(5)(i): (i) All pulpers having the top or any other opening of a vessel less than 42 inches (107 cm) from the floor or work platform did not have such openings guarded by guardrail systems that meet the requirements in 29 CFR part 1910, subpart D, or other equivalent enclosures. For manual changing, openings shall be sufficient only to permit the entry of stock, and shall be provided with at least two permanently secured crossrails, or other fall protection systems that meet the requirements in 29 CFR part 1910, subpart D: Paper Machine #9- Employees were not protected from a fall and drowning hazard where the opening to the Morden pulper, approximately 10 feet, did not have a guardrail installed across the opening.
