29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness. Integra-Cast: Shell Room - In the following instances the employer did not fill out or did not accurately complete an OSHA 301 or equivalent form for each injury or illness case: a) Case #1 - On or about November 09, 2023, and sometime thereafter, when the fingers of an employee were amputated after they got caught in between the running belt of a rotating Mixing Flurry Drum that amputated two of his right fingers, the employer failed to accurately complete the OSHA 301 that identifies an amputation, the correct date it occurred, and the specific object that directly injured the employee. b) On or about December 11, 2023, when an employee sustained a significant injury to the abdomen, after the water pressure from a hose of a wet blast cabinet resulted in a sizeable laceration through his skin, the employer did not fill out an OSHA Form 301 or equivalent for the recordable injury that was under the general criteria recording requirements for the injury that did not result in death, days away from work, restricted work or job transfer, medical treatment beyond first aid, or loss of consciousness occurred. Abatement Certification and Documentation is required.
