29 CFR 1910.213(r)(4): The mention of specific machines in paragraphs (a) thru (q) and this paragraph (r) of this section, inclusive, is not intended to exclude other woodworking machines from the requirement that suitable guards and exhaust hoods be provided to reduce to a minimum the hazard due to the point of operation of such machines. a) On or about February 14, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in the Cutting Department; the Extrema Upcut saw, closest to the office, was not guarded to prevent employee exposure to the shear hazard created by the rotating saw blade. The adjustable and fixed guarding on the saw would not prevent contact with the rotating saw blade when the upcut function was initiated. b) On or about February 15, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in the Cutting Department; the Whirlwind 212L saw, closest to the office, was not guarded to prevent employee exposure to the shear hazard created by the rotating saw blade. The adjustable and fixed guarding on the saw would not prevent contact with the rotating saw blade when the upcut function was initiated. c) On or about February 15, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in the Cutting Department; the Extrema Upcut saw was not guarded to prevent employee exposure to the shear hazard created by the rotating saw blade. The adjustable and fixed guarding on the saw would not prevent contact with the rotating saw blade when the upcut function was initiated. d) On or about February 15, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in the Cutting Department; the Crosscut Solutions Upcut CS18PRH4X saw was not guarded to prevent employee exposure to the shear hazard created by the rotating saw blade. The adjustable and fixed guarding on the saw would not prevent contact with the rotating saw blade when the upcut function was initiated. e) On or about February 15, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in the Cutting Department; the Extrema Upcut saw, closest to the enclosed planer, was not guarded to prevent employee exposure to the shear hazard created by the rotating saw blade. The adjustable and fixed guarding on the saw would not prevent contact with the rotating saw blade when the upcut function was initiated. f) On or about February 14, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in the Ariens Building; the Crosscut Solutions Upcut CS18PL4X saw was not guarded to prevent employee exposure to the shear hazard created by the rotating saw blade. The adjustable and fixed guarding on the saw would not prevent contact with the rotating saw blade when the upcut function was initiated. g) On or about February 14, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in the Ariens Building; the Crosscut Solutions Upcut CSI8PR4X saw was not guarded to prevent employee exposure to the shear hazard created by the rotating saw blade. The adjustable and fixed guarding on the saw would not prevent contact with the rotating saw blade when the upcut function was initiated. h) On or about February 15, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in the Main Cutting Building; the circular saw blade on the panel saw was not guarded to prevent exposure to the rotating saw blade at the end of the guard enclosure immediately adjacent to the operator station. i) On or about February 15, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin, in the Barrel Building; an employee was exposed to potential entanglement hazards from the rotating cutting tooling underneath the damaged shroud on the Baileigh CNC Router table. j) On or about February 2, 2023, at a workplace located at 210 Maple Street Denmark, Wisconsin, in the Ariens Building; the Baker Notcher BN-0 rotating cutting head was not adequately guarded to prevent employee exposure to the shear hazard.
