29 CFR 1910.28(b)(5)(i): The employer did not ensure each employee on a runway or similar walkway was protected from falling 4 feet (1.2 m) or more to a lower level by a guardrail system. Lift Station- Employees were not protected from a fall of approximately 6 feet and a drowning hazard where there was no guardrail along the length of one side of the catwalk over the waste water pit.
