29 CFR 1926.1053(b)(1): Portable ladders were used for access to upper level landing surfaces and the ladders' length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surfaces being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladders: a) On June 19, 2017, employees used job-made ladders to access the third, fourth and fifth floors of a building under construction. The ladders did not extend at least three (3) feet above the landing surface, were not secured at the top to a rigid support, and a grasping device was not provided for employees.
