29 CFR 1910.27(c)(5): The distance from the centerline of the grab bar to the nearest permanent object in back of the grab bars was less than 4 inches. On or about May 12, 2016, the employer failed to ensure that the distance behind the grab bar on the fixed ladder located in Building 155, Hydro Department, Column G:13, was the required distance of 4 inches and not the 1 inch that it was at the time of the inspection, exposing employees to fall hazards.
