29 CFR 1910.119(e)(3)(vi): The process hazard analysis did not address human factors: a) Ammonia Room - Hot gas valve, HV108, located above condenser D1 could not be readily accessed from the condenser platform in an emergency per IIAR 4.3 (e) without having to climb onto condenser D1. The valve was approximately 29" above the condenser and 34" from the side of the condenser. This was not addressed in the 2010 PHA. On or about 2/8/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
