29 CFR 1910.146(k)(1)(iv): When designating rescue and emergency services pursuant to paragraph 29 CFR 1910.146(d)(9), the employer did not inform each rescue team or service of the hazards they may confront when called to perform rescue at the site: On or about June 12, 2025, and at times thereafter, the employer had not designated emergency services where there were hazards associated with manholes and underground vaults prior to entry into those spaces in the following instances listed below: a) On or about June 12, 2025, where one (1) employee was allowed to enter and work in an underground vault; and b) On or about June 13, 2025, where two (2) employees were allowed to enter and work in an underground vault (Vault (53+60)).
