29 CFR 1910.146(d)(4)(i): Permit-required confined space program (permit space program). Under the permit space program required by paragraph (c)(4) of this section, the employer shall provide the following equipment (specified in paragraphs (d)(4)(i) through (d)(4)(ix) of this section) at no cost to employees, maintain that equipment properly, and ensure that employees use that equipment properly: Testing and monitoring equipment needed to comply with paragraph (d)(5) of this section; a) Located at 2734 Home Road, Grove City, OH 43123: On or about November 13, 2024, the employer did not provided testing and monitoring equipment to evaluate a section of HVAC duct, a permit-required confined space, for atmospheric hazards associated with the use of a chemical adhesive used for the repair of insulation inside of the space. The employer had not developed and implemented means and procedures for providing and utilizing monitoring equipment prior to entry into the section of HVAC duct.
